COBRA continuation coverage (U.S. Department of Labor): https://www.dol.gov/general/topic/health-plans/cobra

This template is a starting point for a conversation and a dated record, not legal advice. Employment rules differ by state and country; if you are considering ending someone's employment, check with an employment lawyer or your state labor department first.

In LVL Up, offboarding runs from a checklist you can save as a template, kept on the person's record next to their reviews, 1:1 notes and any improvement plan.

Common questions

Is anything I type saved or sent to LVL Up?

No. The checklist is built in your browser and is gone when you close the tab. Printing, copying and downloading all happen on your device.

When is the final paycheck due?

It depends on your state. The U.S. Department of Labor says federal law does not require employers to give former employees their final paycheck immediately, but some states do. Look up your state's rule through its labor office before the last day.

Do I have to offer COBRA?

The U.S. Department of Labor says COBRA generally applies to group health plans of employers with 20 or more employees in the prior year. Many states have their own continuation laws for smaller employers. Ask your plan administrator or broker which applies to you.

When should I turn off their access?

For a resignation, on the last day. When you are letting someone go, during or right after the conversation, so nobody is left holding the alarm code or the records login. The checklist has a line per system for who removed it and when.
