This template is a starting point for a conversation and a dated record, not legal advice. Employment rules differ by state and country; if you are considering ending someone's employment, check with an employment lawyer or your state labor department first.

How to use it in the meeting

  1. Send the self-review page a few days ahead and read it before you meet.
  2. Fill in your ratings with one example each before the meeting, not during it.
  3. Spend most of the time on the development plan: what happens next, and by when.
  4. Keep the signed copy. Next time, start from this one.

In LVL Up the review, the goals it covers and every 1:1 and piece of feedback since live in the same record per person, so next year's review starts with the evidence already there.

Common questions

Is anything I type saved or sent to LVL Up?

No. The form and the document are built in your browser and are gone when you close the tab. Printing, copying and downloading all happen on your device.

What rating scale does the template use?

Five points: 1 — Not meeting expectations; 2 — Partly meeting expectations; 3 — Meeting expectations; 4 — Exceeding some expectations; 5 — Consistently exceeding expectations. Every rating asks for one example, because a number without an example is the part people argue about.

Should the employee fill in a self-review?

It helps. Tick “Employee self-review” and give them that page a few days before the meeting. Reading their view first makes the conversation shorter and less of a surprise for both of you.

Does the employee have to agree with the review to sign it?

The sign-off line in this template says the signature means they received the review, not that they agree. There is room for their comments. Whether a signature is needed at all is your choice.
