Is anyone overdue for a review, or about to lapse?
Type your staff below with the date of each person's last review and the expiry date of one license or certification. Press the button and you get a list of who is overdue, who is coming due, and whose credential expires in the next 60 days. It works with no account. Nothing you type leaves your browser.

How this is computed
Review overdue
A person with no review on record, or whose last review was more than 365 days ago. Annual reviews are the norm in a small practice, so a year is the line.
Review due soon
A last review more than 300 days ago but not yet 365. That gives about two months to schedule it before it is overdue.
Credential expired
An expiry date before today. The report says how long ago it lapsed.
Credential expiring soon
An expiry date within the next 60 days, including today. Most state license renewals and re-certifications can be completed in that window if you start now.
Dates are compared by calendar day. A date the tool cannot read is left out of the counts and marked on that person's line.
What stays private
The table and the report exist only in your browser. No row is sent to LVL Up Performance or to anyone else, and nothing is stored when you leave. The one optional form, “Send me the practice HR checklist”, sends your email address and the five counts from the report. It never sends a name, a role or a date.
What to do next
Schedule the overdue reviews first; they are the ones a person has noticed. Then start renewals for anything expiring soon, because the person holding the credential usually has to act. If this check found something, it will find something again next quarter unless the dates live somewhere that alerts you.

Common questions
Is anything I type saved or sent to LVL Up?
No. The table and the report are computed in your browser and are gone when you close the tab. The only request the page can make is the optional checklist form under the report, and that sends your email and the five counts, never a name, role or date.
How often should a small practice review staff?
This tool treats a review older than 365 days as overdue, because an annual review is the common cadence in owner-run practices. Some practices add a shorter check-in at 90 days for new hires; the tool does not model that.
Does the tool verify licenses with the state board?
No. It only does date arithmetic on the expiry date you enter. It does not connect to any licensing board or perform primary-source verification, and neither does LVL Up Performance.
What if a person has more than one credential?
Add a second row for the same person with the other credential. The tool checks one credential per row; the report will list the person twice, once per credential.
